Request access to County EHR systems

If you are provider joining the BHSD network, you will need to request access to County EHR system by completing the forms below.

Remote access request (Attachment A)	Secure your supervisor's approval for the remote access request. Specify your mobile device type, e.g., iPhone or Android. Provide an email address that you can access on your mobile device. Submit this with the BHSD system access request form (SARF) above.
IT user responsibility statement	Submit with your remote access request. Submit this with the BHSD system access request form (SARF) above.
Electronic signature agreement	Maintain this document in your records for auditing purposes.
