5150 Training

5150 refers to an involuntary evaluation. Section 5150 is a section of the California Welfare and Institutions Code (specifically, the Lanterman–Petris–Short Act or "LPS") which allows a qualified officer or clinician to involuntarily confine a person deemed to have a mental disorder that makes them a danger to him- or herself, and/or others and/or gravely disabled. 

5150 Training Registration Process – Current trainings are provided on Zoom

The following requirements must be met to attend the 5150 Training: 

  1. All applications need to be submitted ten business days before the desired training date - No walk-ins allowed!
  2. You must have successfully submitted your 5150 Authorization Training request in DocuSign and be pre-registered in Zoom to participate in the training.
  3. Both visual and audio participation are required throughout the training. Please test your computer/device for Zoom use prior to registering.
  4. Please check-in at least 15 minutes prior to the start-time and wait to be admitted and ensure that audio, visual and media equipment are connected and that your internet connection is working properly.

5150 Testing Process 

  1. You will need to take the test after the training at Learning Partnership, 1075 E. Santa Clara St. 2nd floor, San Jose. There is no remote or on-line test taking.
  2. You must pass the test with 80% or higher to receive your 5150-authorization card. Your 5150 card will be sent to you in DocuSign. 
  3. If you do not pass the test on your first attempt don't worry, you will have two more attempts.

Registration Process:

Prior to beginning the application process you will need the following:

  1. Please do a DCA Search and attach it to your application where it says attach Breeze
  2. PDF of your actual license to attach to the application
  3. Email address and phone number of your supervisor
  4. Work Email address of your Director or Designee

Once you submit your application the following will take place:

After your supervisor and director signs your application in DocuSign, Learning Partnership will process your application. You will then receive an email from DocuSign saying your document is complete. You will need to open your application to see the training date that you were approved for.  Cards are valid for five years. Issuing 5150 cards is completely at the discretion of the County of Santa Clara Behavioral Health Services Department.  Admittance to the class does not guarantee a card.  We reserve the right to take a card at any time.  

If you are an Associate Level staff, Psychiatric Technician, Licensed Vocational Nurse, or Unlicensed staff, you need to provide a letter from your immediate supervisor requesting the exception which includes:

  1. Why is this exception needed by your organization?  Please explain why a licensed staff cannot write these holds.
  2. The experience and education which makes the applicant qualified to write a 5150 hold.
  3. Documentation about who the applicant can call 24-hours a day for supervision.  This must be a licensed person, who has a current 5150 card.  This includes their phone number, name, 5150 card expiration date and license number/type.

Updating Agency Information for Current 5150 Card holders:
The 5150 Authorization Card is only valid when used in the performance of duties for the agency that is listed on your 5150 card. If you are a current 5150 cardholder and begin to work for a new agency you need to notify Elisa Perales at [email protected] and submit a new application form from the new agency. However, you do not need to retake the training. Once your application is updated, a new 5150 card will be emailed to you.  Please note that using a 5150 card from another agency is not permitted.

Zoom 5150 Trainings are from 9:00 am to 12:00 pm.

After the training, participants will need to take the test in-person at the Learning Partnership office, 1075 E. Santa Clara St. 2nd floor, San Jose, 95116. The test must be taken the same day as the training at one of the following times:

  • 1:00 – 2:00 pm
  • 2:00 – 3:00 pm
  • 3:00 – 4:00 pm
  • 4:00 – 5:00 pm
