5150 refers to an involuntary evaluation. Section 5150 is a section of the California Welfare and Institutions Code (specifically, the Lanterman–Petris–Short Act or "LPS") which allows a qualified officer or clinician to involuntarily confine a person deemed to have a mental disorder that makes them a danger to him- or herself, and/or others and/or gravely disabled. 

5150 Training Registration Process – Current trainings are provided on zoom

The following requirements need to be met to attend the 5150 Training: 

  1. All applications need to be submitted ten business days before the desired training date - No walk-ins allowed!
  2. You must have successfully submitted your 5150 Authorization Training request in DocuSign and be pre-registered in Zoom to participate in the training.
  3. Both visual and audio participation are required throughout the training. Please test your computer/devise for Zoom use prior to registering.
  4. Please check-in at least 15 minutes prior to the start-time and wait to be admitted and ensure that audio, visual and media equipment are connected and that your internet connection is working properly

5150 Testing Process 

  1. You will need to take the test after the training at Learning Partnership, 1075 E. Santa Clara St. 2nd floor, San Jose. There is no remote or on-line test taking.
  2. You must pass the test with 80% or higher to receive your 5150-authorization card. Your 5150 card will be sent to you in DocuSign. 

Registration Process:

Prior to beginning the application process you will need the following:

  1. Please do a DCA Search and attach it to your application where it says attach Breeze
  2. PDF of your actual license to attach to the application
  3. Email address and phone number of your supervisor
  4. Work Email address of your Director or Designee

Once you submit your application the following will take place:
After your supervisor and director Docu-Signs your application, Learning Partnership will process your application. Then, you will receive an email saying your document is completed. You will then need to open your application to see the date of your 5150 class acceptance. Print or have available on your phone the document approval with you when you arrive for the training. Cards are valid for five years.

TEMPORARILY, WE ARE NOT ACCEPTING ONLINE  APPLICATIONS FOR UNLICENSED PROVIDERS.

If you are unlicensed or a Psychiatric Technicians you need to complete a different application, which requires a PDF attachment from your supervisor requesting the exception.  Your application must have a letter attached from your supervisor, which includes:

  1. Why is this exception needed by your organization?  Please explain why a licensed staff cannot write these holds.
  2. The experience and education which makes the applicant qualified to write a 5150 hold.
  3. Documentation about who the applicant can call 24-hours a day for supervision.  This must be a licensed person, who has a current 5150 card.  This includes their phone number, name, 5150 card expiration date and license​ number/type.

​Issuing 5150 cards is completely at the County of Santa Clara’s discretion.  Admittance to the class does not guarantee a card.  We reserve the right to take a card at any time. 

Updating Agency Information for Current 5150 Card holders:
The 5150 Authorization Card is only valid when used in the performance of duties for the agency that is listed on your 5150 card. If you are a current 5150 cardholder and begin to work for a new agency you need to notify Jeannette Ferris at [email protected] and submit a new application form from the new agency. However, you do not need to retake the training. Once your application is updated, a new 5150 card will be emailed to you.  Please note that using a 5150 card from another agency is not permitted.

Zoom 5150 Trainings are from 9:00 am to 12:00 pm
After the training, participants will need to take the test in-person at the Learning Partnership office, 1075 E. Santa Clara St. 2nd floor, San Jose, 95116.

  • 2:00 – 3:00 pm

Additional testing date – May 20th at Kaiser. In efforts to increase the number of training slots for the May 19th 5150 training we added May 20th as another day to take the 5150 test.
The address for the May 20th test taking site is 19000 E. Homestead Road, Building 1, Floor 1, Cupertino, CA 95014 (Cross streets are Tantau and Homestead). Testing dates and location will be offered first come first serve basis.
When entering the Kaiser building all coming will be screened by a greeter for symptom screening, hand sanitizing, and temperature checks. Visitors will then proceed to Group Room A, where the 5150 testing will take place.  Free parking is available on-site.
