The Client Status Report (CSR) replaced the Treatment Status Report (TSR) in 2020. This Information Bulletin is being distributed to remind all providers that the CSR should be used to communicate with courts & other entities that require a status report.
Please note that the CSR is used in the same manner as the TSR.
The purpose of the CSR is to communicate client status to the referring court or agency, provided all appropriate releases and consents are in place. The CSR should be sent monthly to the referring court or agency. If there is a significant change in the status of a participant, a CSR can be sent at any time during the month.
The CSR provides information on:
• Client participation in treatment
• His/her attendance and engagement in treatment, and
• Changes to the treatment plan which have been incorporated during the reporting month.
• Changes to the treatment status and level of care that have occurred during the reporting month.
The CSR is posted on the BHSD website at the page shown below, under Treatment Services, and can be downloaded in both PDF and WORD formats.
